The game will highlight the requirements when you select the upgrade component that is incorporated into the item. These include the item type (either Weapon or Armor) the level of the item and the Upgrade Token required, and the amount of progress needed to complete the upgrade.
Once you've met the required requirements for upgrading, the game will place the item in your inventory and the progress bar will update to show how close to completing the upgrade. You can also check the status of an upgrade by visiting any NPC that upgrades items.
